  1. The parties agree to share joint legal custody of this minor child, and this joint custodial arrangement requires the parties to communicate and discuss any major issues regarding the health, education and religious upbringing of this child. Each party will be responsible for half of the un-reimbursed medical expenses. Non-emergency medical/dental procedures (including but not limited to braces) not covered by health insurance are to be discussed and agreed upon ahead of time.
  2. Defendant shall be entitled to visit with the parties' child on alternating weekends from 6:00 pm Friday until 6:00 pm Sunday. These alternating weekends shall commence as of ???, and shall alternate thereafter. Parties shall exchange the child at _____ location.
  3. Day/s of the week - to be defined.
  4. Summer vacation. Each party shall have the child one full week during the summer. Defendant may not remove the parties' child from the State of Pennsylvania without prior notification to the Plaintiff. The Defendant shall be responsible for daily phone contact to the Plaintiff between the hours of 5-7 each evening.
  5. The parties shall alternate all major holidays Mother even years, Father odd years.

    ix.Christmas Day - (MOTHER)12:00 noon until 8:00 am the next day
    x. Other major holidays to be specifically defined.
  6. Where a Monday holiday is adjacent to a parties' week-end, the visitation will continue through and include Sunday Monday night.
    (1)Holiday hours are defined as: 9:00am - 7:00 pm.
    (2) Defendant shall have holiday visitation with the parties' child on Defendant's birthday, each Father's Day, the Plaintiff shall have holiday visitation with the child on Defendant's birthday and Mother's day.
    (3) The Mother shall have the parties' child on her birthday until she reaches the age of 7, at which time the parties shall alternate with the Defendant commencing on June 1, 2003.
  7. Should the Defendant fail to comply with any provisions of this visitation schedule, then upon Plaintiff's ex-parte application, with notice to Defendant, a bench warrant shall be issued for the arrest and incarceration of Defendant/Plaintiff, and an Order transferring custody of the parties' child to Defendant/Plaintiff _____.
